LIABILITY RELEASE: I, (must be 18 years or older to complete this form and enroll in class) wish to enroll my dog (listed in this registration form) in obedience training classes under the direction of Blazing Trails K9 Academy. I understand that my success or failure in this program depends upon my ability to practice with my dog 20-30 minutes a day as explained by the instructor, and that no guarantees are placed upon the dogs training or behavior as a result of my completion of this class. I also understand that the class will only cover the tips on teaching the basic on leash obedience commands of sit, stay, come, loose leash walking, and down, as well as tips on very basic problem solving, and socialization skills. As the legal owner of my dog or acting in behalf of the legal owner (must have legal owner accept. I understand that my enrollment in dog obedience group classes is not without risk and having read and fully understanding this agreement, I do hereby waive and release Blazing Trails K9 Academy and Laurelwood Kennels, and any owners, agents, leaseholders or landlords of any location where group classes are taught, from any and all liability of any nature. This includes any injury, death, illness, or damage my pet, myself, or my property may suffer before, during, or after training. This also includes any guests I bring with me to class. I also indemnify and hold harmless Blazing Trails K9 Academy and Blazing Trails K9 Academy and Laurelwood Kennels, and any owners, agents, leaseholders or landlords of any location where group classes are taught, from any claims to any damage my pet may cause to any person or property before, during, and after training in this course. I understand that all fees are payable to reserve my space in class (class size is limited). I agree and understand that upon entering the first class (orientation) there are NO REFUNDS. I also understand that the unlimited lifetime comebacks offered with this course are dependant upon courses being offered and that there is a minimum of 3 paid students required in group class for that group class series to begin as scheduled. I also understand that there will be a maximum of 3 repeat students allowed at any class and if the maximum is reached when I want to return, I may be asked to wait for another series of classes in order to repeat. Any person that reserves his or her space in the class understands that if they fail to attend the first session of class and do not cancel their enrollment through Blazing Trails K9 Academy, is not entitled to a refund, but may attend the course at a later scheduled class time. Unlimited lifetime comebacks are limited by the number of classes offered. We will maintain your record on file and as long as we are having classes at any of our locations, you are welcome to attend by calling to let us know you will be coming to make sure there are enough spots in that class for repeats. I also understand that Blazing Trails K9 Academy, it’s designated instructor,Blazing Trails K9 Academy and Laurelwood Kennels, and any owners, agents, leaseholders or landlords of any location where group classes are taught, reserves the right to refuse admission to the class or to remove a student from the class at any time. I understand that should my dog display any aggressive tendencies towards another dog or another person during the class, it is my obligation to inform the instructor immediately. Individual private in home lessons may be required in conjunction with the class in order to continue my class enrollment. This liability release will remain on file for this and future class enrollments and all parts shall remain in full effect unless there is a written request signed by both parties (Blazing Trails K9 Academy and the enrolling student) to change this agreement.
